Application Instructions:

After downloading the file open it with the Adobe Reader.

The document is two legal (8.5x14) pages. You will need to print the application in two steps:

• Before you print the application click on the File Menu and then click on Page Setup, when the setup screen appears set the page size to 8.5x14 then print page one.
• When finished turn the page over and then place it upside down in the printer. We want page two to be printed on the backside of page one, when ready print page two. (Note: This works for most inkjet printers)

IMPORTANT! We'll only accept applications that are on legal paper and printed on one page (front and back). If you don't have the means to print the application just give one of our adoption representatives an email and they'll be happy to send one out to you.
